Home healthcare provides medical and daily health-related services to people in their homes. Home healthcare services can be used to provide not only short-term care for hospital and nursing home-discharged patients, but it is also used for people with ongoing health concerns and issues.  At SRH, we offer what is called In-Home Concierge Care. We assess each case on an individual basis and customize services according to the needs of the client. Programs include; Skilled Nursing Services, Traumatic Brain and Spinal Cord Injury, In-Home Alzheimer’s and Dementia Care, Personal Care Assistance, Care Coordination, Home Companion Services, and Medication Management Services.

Skilled Nursing Services - A licensed health professional (Registered or Licensed Practical Nurse) who is supervised by a physician to provide care for the sick or infirmed.  The skilled nurse promotes and maintains health through assessment, evaluation, instructions, and treatment in an attempt to assist patients in their own homes and is part of the collaborative care team.

Personal Care Assistant - Provide nutritional support, assistance with personal hygiene, daily living activities, protective oversight, and environmental maintenance to acutely ill and/or chronically ill patients in their homes.

Homemakers - Provide meal preparation, light housekeeping activities, and related home activities to acutely ill and/or chronically ill patients in their homes.

In-Home Alzheimer’s and Dementia Care - 

When a loved one is living with Alzheimer’s disease, one of the most difficult parts is ensuring the patient’s safety while allowing them to remain as independent as possible.
